Neonatal follow-up services
support for infants and children at higher risk of developmental delay due to prematurity or birth complications * in partnership with Mackenzie Richmond Hill Hospital, Markham Stouffville Hospital and Southlake Regional Health Centre * referral from physician required

Inclusion support services
supports children aged birth to 13 years attending licensed child care, licensed before and after program or approved recreation program in York Region * supports and visits Child Care Centre and supports family with service coordination * general classroom consultations * child-specific program planning * professional development for child care staff

Therapy services
information and referral for children who require occupational therapy (OT) and physiotherapy (PT) * funded by York Region and Children’s Treatment Network (CTN) * individual and group services * eligibility determined by occupational therapist or physiotherapist

Application Parent direct referral by calling the KIDS Line * physicians and other professionals can refer with parental consent by faxing ERIK or Nipissing District Developmental Screen and referral form from website to Intake 905-762-2115 * no walk-in service to service locations
Languages English ; Interpretive Services - Access York ; provides interpreters in any language for face to face service and via a language line over the phone

Site Location Services are delivered in 6 offices across the Region, phone Intake is only in one office * There is no face to face or walk-in Intake service at the site where phone Intake is conducted
