Description (Service) Provides primary healthcare, health promotion, chronic disease prevention and management services, and community-based programs to meet the needs of residents using a multi-disciplinary staff team approach * Satellite Location for Community Legal Clinic NOT a walk-in clinic ;

Chronic Disease Prevention Programs include

Diabetes Education group of health care professionals from different areas of specialization support and assist to manage diabetes * individual counselling and group sessions * Workshops led by a nurse and dietitian cover a range of topics such as healthy eating; exercise; risk factors and complications of diabetes; food label reading; stress management

COPD Program Registered Respiratory Therapist/Certified Respiratory Educator works one-on-one with individuals in educational session to provide skills and knowledge to improve the management of your COPD

Physical Activity Program offers one-on-one exercise assessments, prescriptions and customized exercise plans and group exercise sessions Registered Kinesiologist and Exercise instructors * offers free programs by partner agencies, programs held by centre and space requests from community such as Tai Chi for Health and Light Exercise for Seniors * see website for full list of programs and descriptions
