Description (Service) Provides community outreach, housing, medical and addiction services, financial support, social and peer support, public health workers and a team of mental health specialists * individuals are able to receive support for basic needs and potentially life-saving services with self-worth and dignity * entry point to services;

The Hub offers drop-in support for clients and individuals referred by other agencies, as well as support services for outreach van clients who voluntarily request more extensive support that the van cannot provide * supports client activities such as meal programs, a writing group, and a variety of community-building activities;

Crosslinks Housing and Support Services offers a range of housing options including individual and shared apartments and shared houses, and individual assistance in finding suitable temporary or permanent housing ;

Case Management services include assessment of needs, development of goals, acquisition of life and problem solving skills, crisis intervention and prevention, advocacy and linking with other community, social and health care services and resources;

Mental Health and Justice Program offers specialized services for people who are in conflict with the criminal justice system * provides supportive housing and case management to individuals with mental health and addiction challenges and histories of homelessness who are in conflict with the criminal justice system;

First Episode Program shared, supportive housing for young people who are experiencing their first psychotic episode, usually between the ages of 16 and 26 * youth are able to receive specialized, recovery-focused supports while remaining close to home and the natural support system provided by family, friends, work and school;

Addictions Supportive Housing provides specialized supportive housing for individuals who are homeless and are experiencing chronic complex addition challenges, or the combination of substance abuse and mental health issues ;

The Reintegration Housing Program is transitional housing to help people successfully transition back into the community ;

Street Outreach Van (NKT0714) meets clients at convenient locations throughout the region
