Description (Service) specialized services for people who are homeless, at imminent risk of becoming homeless, those involved with the criminal justice system, young people who are experiencing first episode of psychosis and people with substance use challenge

medical and addiction services * financial support, social and peer support * public health workers and a team of mental health specialists * support for basic needs * intake and case management ;

Community Case Management mental health and addiction case management

Mental health supportive housing housing options for individuals who struggle to find and maintain housing * all housing affordable and includes support from case manager or housing support worker: *Reintegration housing: 24hr staffed house with intensive support * two units for stay up to 6 months for individuals who have reoccurring hospital visits
*First episode: housing for individuals 16-24 experiencing symptoms of psychosis for first time
*Mental health and justice: housing for individuals involved in criminal justice system
*Addictions Supportive Housing (ASH): partnership with Addiction Services York Region (ASYR) * housing for individuals with chronic complex addiction
*Complex needs: housing for individuals with 4 or more comorbidities who require extra support

Peer Led Drop-in twice week drop-in service * space to socialize, use computer, have access to snacks and shower

Street Outreach Van mobile daily service * mental health and nursing assessment/care, crisis support, intervention and prevention, condoms, needle exchange, assistance in finding shelter and transport for first-time access to shelter, survival supplies like food, clothing, sleeping bags, tarps and tents * meeting time and place arranged with caller
