Application Accepts applications from Canadian citizens or residents of Canada who are visually-impaired and in good general health and whose lifestyle would be enhanced by having a Guide Dog * application forms are available online or by calling the National Office

Fees Users lease a Guide Dog for $1.00 and are responsible for the care, feeding, and veterinary costs for the dog
