Description (Service) Supports for older adults and adults with disabilities * short term hospital to home care * helps people remain independent in their own homes for as long as possible * hourly, overnight, or 24-hour care * custom care plans, such as end-of-life care, hospital-discharge care * care associated with chronic conditions: 
companionship, homemaking, and errands * medication administration or reminders * nursing and therapy * personal care and hygiene*physical assistance with changing positions, moving around, getting dressed, and home exercise programs * senior wellness coaching * transportation and escorts to medical and dental appointments*palliative care

Office hours: Mon-Fri 9am-5 pm * closed 12noon-1pm
