Description (Service) A resident of Ontario must have a health card to show that he or she is entitled to health care services paid for by OHIP

Eligibility Ontario residents are eligible for provincially funded health coverage (OHIP) * generally, to be eligible for Ontario health coverage you must: be a Canadian citizen, permanent resident or among one of the newcomer to Canada groups who are eligible for OHIP as set out in Ontario‚Äôs Health Insurance Act ; and be physically present in Ontario for 153 days in any 12-month period; and be physically present in Ontario for at least 153 days of the first 183 days immediately after establishing residency in the province; and make your primary place of residence in Ontario 
Healthy Smiles program - children and youth 17 years old and under from low-income households * cosmetic dentistry including teeth whitening and braces are not covered * automatically eligible for Healthy Smiles Ontario if they or their family receive Ontario Works, Temporary Care Assistance, Ontario Disability Support Program, Assistance for Children with Severe Disabilities  
Children are eligible for the program if they are 17 years of age and under, live in Ontario, come from a household with an income equal to or less than the levels in the table online
Application To renew your health card you will need: your health card; your renewal reminder - this reminder is sent to you by mail 2 to 3 months before your health card expires; proof of your Canadian citizenship, Ontario residency and personal identity - check the Ontario Health Coverage Document List (form 9998E-82) for acceptable documents; the Health Card Renewal Form that is printed on the back of your renewal reminder - if you have lost your renewal reminder, you can print another copy of the form 
Healthy Smiles: enrol in the program online or by mail, will get a Healthy Smiles Ontario dental card in the mail. that must be presented to dental provider at each visit
