Description (Service) Services include: 
Community and in-home services -- nursing care, physiotherapy, occupational therapy, speech-language pathology, social work, personal support * if eligible for above services, other services may be provided such as drug card, medical equipment or supplies, laboratory services 
Child and family services -- for children or youth with short or long-term disabilities or other special needs and their families * in-home health and personal support services as listed above, with the addition of palliative care and respite care (caregiver relief) * school-based services -- professional services to enable children with disabilities or ongoing diseases to attend school 
Acquired brain injuries -- access to and coordination of services 
Also offers information and assistance in linking to existing community support services, which may also be contacted directly by consumers * such services include friendly visiting, meals on wheels, volunteer transportation, security checks, that are relatively low in cost and primarily volunteer-based * also help finding other services, such as attendant care for persons with physical disabilities and supportive housing services 
Placement in long term care homes -- access for admission to long term care homes, or for transfer to facilities outside Toronto * crisis placements * subsidies for basic (ward) accommodation may be available -- apply directly to facility after placement * for seniors apartments contact Housing Connections – see separate record 
Short stay -- respite for caregivers * includes persons with Alzheimer Disease * Convalescent Care -- following surgery or illness * medical and therapeutic supportive care provided in a long term care facility * usually 45-55 days 
Adult Day Programs -- Central CCAC manages access to adult day programs * Care Coordinator will assess health care needs, provide information on adult day programs in the community and discuss the programs that best meet personal needs * will also assist with the admission process 
Assisted Living Services -- Central CCAC manages access to assisted living services * Care Coordinator will assess health care needs to determine eligibility, provide information on Assisted Living Services and work with clients to determine which services will best meet their needs * will also assist with the admission process

Eligibility Must have Ontario Health Insurance 
In-home health and social services -- Need for professional or support services in the community (home or school) that cannot be met on an outpatient basis * those needing service may include adults and children with any illness, seniors, persons with physical, psychiatric or developmental disabilities 
Placement in long term care homes -- Adults 18 years and older who require placement in long term care homes * must meet one or more of the following: need availability of 24 hour nursing care, or assistance with daily living, or on site supervision, and other caregiving and community services no longer sufficient 
Adult Day Program -- Adults 18 years of age or older who possess a valid Ontario Health Card and have a health need that can be met through an adult day program 
Assisted Living -- Applicants must: possess a valid Ontario Health Card, be assessed by a CCAC care coordinator, be a senior with care needs that require the availability of 24-hour personal support or attendant services, not require 24-hour nursing services, and live in a building where Assisted Living Services are provided or within a specific geographic boundary of an approved Assisted Living service provider

Placement in long term care homes -- eligible applicants will be placed on waiting list of up to 5 facilities in Ontario chosen by the applicant * applicants will not be placed without their consent * applicants may select individual facilities, but may not apply to them directly
