Description (Service) day program for individuals with Alzheimer's Disease and related dementias * Structured daily activities include indoor and outdoor exercises, baking, music programs, mental stimulation, discussion groups, reminiscing programs, gardening, projects (woodworking, sewing, knitting, quilting), and games * Foot care clinic every 6 weeks * Hair care * Daily hot meal and two snacks provided * Escorted transportation * respite care;

Thornhill location: 10 Harlech Court, Unit 2, Thornhill, ON, L3T 6L5 Stouffville location: Parkview Village, 12184 9th Line, Whitchurch-Stouffville,L4A 3N6 Family support group provides information and support on coping with and caring for people with Alzheimer disease and related disorders
Hours Aurora and Thornhill Mon-Fri 7 am-6 pm, Sat 8 am-5 pm * Stouffville Mon and Wed only 9 am - 5 pm

Eligibility Diagnosis of Alzheimer’s disease or related disorder * living within the Region of York * meets CCAC (Community Care Access Centre’s) eligibility criteria
Application Referrals are welcomed from hospitals, health and other social service agencies, physicians, relatives and friends, self-referrals

Fees Program - $22 per day ; Fee assistance available for individuals with financial need if eligible ; assistance is provided by CCAC.
