Description (Service) Family health care services by nurses for people without primary care provider * health services including Chronic disease management (such as diabetes, asthma, high blood pressure), Prenatal care, Healthy baby and child care, Physical check-ups, Diagnosis and treatment of acute illnesses, infections and injuries, Writing prescriptions, Immunizations, Foot care program, Palliative care, Laboratory services, Counselling services, Nutrition services, Physiotherapy services, Pharmacist services, Cancer screening

Diabetes Conversations group program that allows you to learn and talk about diabetes in a fun and interactive way, four weekly sessions that cover diabetes-related topics
Healthy You Program all-in-one 10 week program gives practical information on healthy eating, physical activity and feelings around lifestyle, as well as helpful tools to help maintain your healthy lifestyle
Mind Your Mood monthly support group for individuals living with anxiety and depression to implement wellness strategies and improve quality of life
Smoking Cessation individual assessment, counselling and coaching to quit, nicotine replacement therapies and cessation medications, motivational interventions, resources for quit plan, referral to community resources
Women’s Wellness Group is designed for women to explore female-focused health and wellness for senior women
STOP Program smoking cessation program, cost-free nicotine replacements and counselling * advance registration required * referral by nurse practitioner to participate
GLAD Program Good Life with osteoarthritis in Denmark (GLA:D®) program * for individuals 30 years of age or older and have hip or knee osteoarthritis * 7-week program with education and exercise sessions led by physiotherapist
